PROBE Equipment and Reagent Requests

If you are interested in using PROBE's equipment or reagents for use in your classroom, please download one of the forms, below. Refer here for labs and equipment that are available to trained teachers. (Diabetes Tech will be added soon). Look carefully through the spreadsheet for your selected lab. Please note any major items that you will NOT need when you fill in the request form (ie. On the forms we must have the following information: School, contact teacher, phone number and email address, the number of sections and students, and dates desired for each lab. We will also be happy to provide in-class support, so be sure to include those dates as well, if applicable. Please contact us about special requests.

Email the completed form(s) by September 15th for first semester requests and January 15th for second semester to Karen Santiago, PROBE coordinator.

Note: Equipment is reserved on a first-come-first-served basis. The earlier you schedule, the greater the chance of getting the materials on the date requested.
